ADOPTION AGREEMENT
K-9 ResQ, a volunteer
organization, agrees to place the following described dog/puppy with you, the
Adopter:

You understand that
this dog was previously unwanted or lost and may have been rescued by us from a
dangerous, unhealthy and/or cruel situation.
This could have long-lasting effects on the dog.
You agree that K-9 ResQ is making no representations or warranties about
the condition, personality or temperament of the dog other than our knowledge of
it’s behavior while in K-9 ResQ foster care.
We require an adoption
fee of $95
to help defray our expenses for veterinary care.
Any donation you care to make in addition to the adoption fee is greatly
appreciated.
If the dog is not
already spayed/neutered, you are also required to pay a $50
spay/neuter deposit, which is
refundable when you return a certification from a licensed veterinarian that the
dog has been spayed or neutered. If
such proof is not offered within 30 days of adoption, the dog must be returned
to K-9 ResQ.
You agree to provide
this dog with a safe and loving home for the remainder of its life.
It must receive annual vaccinations and monthly heartworm prevention
available from a licensed veterinarian in addition to other services as
recommended by your veterinarian. If
it is ever found to be mistreated or kept in unsafe or unsanitary conditions,
K-9 ResQ reserves the right to reclaim the dog.
If for any reason you are ever unable to keep the dog, it must be
returned to a K-9 ResQ volunteer and you will be given a full refund of your
adoption fee.
